**Job Summary**
NHS Golden Jubilee is seeking a highly skilled and dedicated Registered Nurse to join our team in the Scottish Advanced Heart Failure Service. As a key member of our nursing team, you will play a vital role in providing exceptional care to patients with advanced heart failure and those awaiting heart transplantation.
**Key Responsibilities**
- Work collaboratively with the transplant coordinators to deliver high-quality patient care in a clinic setting.
- Develop and implement individualized care plans to meet the unique needs of patients with advanced heart failure.
- Contribute to the development and implementation of nursing policies and procedures to ensure best practice in heart failure and transplant care.
- Participate in multidisciplinary team meetings to discuss patient progress and develop comprehensive care plans.
- Provide education and support to patients and their families to promote self-management and independence.
**Requirements**
- NMC registration as a Registered Nurse.
- Excellent time management and organizational skills.
