Job Summary

We are seeking a highly skilled Care Coordinator for HIUs to join our team at Central London Community Health Trust. As a key member of our Urgent Care Service, you will play a vital role in coordinating and delivering support services to High Intensity Users (HIUs) of our Urgent and Emergency Care services.

Main Responsibilities

* Identify HIUs who require additional support and review their needs with relevant clinical and other support teams

* Determine the most suitable services to provide support and refer patients accordingly

* Track processes and outcomes of the service and provide information and training to local health and social care professionals

* Coordinate attendance at MDT meetings, record accurate minutes, and work with local providers to prioritize the needs of HIUs

* Maintain accurate records on the HIU Patient Case Tracker and use the HIU dashboard to track patient activity

* Develop and maintain effective working relationships with clients and the wider team

* Work with a wide range of health, social care, and third sector organizations to prioritize care for HIUs

* Facilitate discussions between primary care, hospital, and community services

* Manage patient caseloads and advise on improvements to deliver measurable outcomes

* Publicize the service to users and staff across the Borough and work with local organizations to prioritize care for HIUs

Requirements

* English and Maths GCSE

* Specialist knowledge or interest acquired through training and extended courses related to healthcare

* Excellent IT skills and positive attitude towards learning and using your own initiative for problem solving

* Proven record of excellent written and verbal communication and interpersonal skills

* Good organizational and time management skills

* Ability to prioritize and manage own workload and show initiative to complete tasks independently and with multi-disciplinary teams

* Experience of administrative duties and working in a clinical healthcare setting

* Ability to establish credibility and develop and maintain effective working relationships with clients and the wider team

* Strong analytical and judgement skills and ability to interpret information and present results in a clear and concise manner

* High level of self-motivation, drive for performance and improvement, and flexibility in approach

* Willingness to undergo further training or development
