About this role:

Clinically qualified professionals are sought to join Bush & Co as Associate Clinical Case Managers. As a key member of our team, you will manage a case load of clients through their rehabilitation, leading on assessment in the home and goal setting. You will signpost and advocate for the needs of those you support, assessing what needs to be in place for them to live a fulfilled life.

Key responsibilities:

  • Manage a case load of clients through their rehabilitation
  • Lead on assessment in the home and goal setting
  • Signpost and advocate for the needs of those you support
  • Manage all administration in relation to cases

Requirements:

  • Proven clinical experience and a valid clinical registration
  • Previous experience putting together and implementing rehabilitation packages
  • Strong relationship building skills
  • Ability to travel across the UK
