Job Summary:

We are seeking an experienced Occupational Therapist to join our team as a Care Assessment Professional. This role involves conducting thorough assessments of referred individuals to identify their care and support requirements.

Key Responsibilities:

  • Conduct assessments of referred individuals to identify their care and support requirements.
  • Develop and implement individualized care plans to ensure each person's needs are met.
  • Maintain accurate and up-to-date records of assessments, care plans, and progress.
  • Work collaboratively with our multidisciplinary team to provide comprehensive support to individuals with complex needs.
  • Identify and mitigate risks associated with each individual's care plan.

Requirements:

  • Registered Occupational Therapist (OT) or equivalent qualification.
  • Experience working with individuals with complex needs, including mental health, learning disabilities, and physical disabilities.
  • Strong assessment and communication skills.
  • Ability to work independently and as part of a team.
