Advanced Community Nursing Leader

Job Summary
We are seeking an experienced and skilled Registered Nurse to take on a leadership role within our Integrated Care Team. As a Senior Registered Nurse, you will be responsible for managing a designated group of patients, providing nursing assessments, diagnoses, treatments, and advice, and conducting general assessments for the team.
The ideal candidate will have a strong understanding of community nursing and experience working with patients with frailty. They will also possess good IT skills and be familiar with Home First/D2A approaches. Key Responsibilities
- Lead and manage a team of nurses to deliver high-quality patient care
- Conduct comprehensive assessments, develop care plans, and evaluate their effectiveness
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Participate in staff development, appraisals, HR processes, clinical supervision, and promotion of staff development ideas

Salary and Benefits Package
The salary for this role is estimated to be around £40,000-£50,000 per annum, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including pension scheme, life insurance, and generous annual leave allowance.
