About the Role

We are seeking a highly skilled and experienced Clinical Assessor to join our Digital Care Hub team, supporting the delivery of the new Harrogate Goldline Service. This is a unique opportunity to be part of a pioneering service, utilizing telemedicine to provide high-quality care to patients in their own homes.

Key Responsibilities
  • Provide remote clinical assessments, establishing care plans for patients accessing the Goldline service, in accordance with Trust guidelines and protocols.
  • Support and care for palliative/end of life patients and their families, via telephone.
  • Participate in early intervention, assessment, observation, and treatment for patients, utilizing clinical skills and knowledge.
  • Ensure timely responses to remote monitoring alerts.
  • Work with individual care plans to provide personalized care, referring to other services as needed.
  • Collaborate with acute, primary care, and community teams in managing sick patients.
  • Participate in in-service training, preceptorship, and supervision of junior staff, post-basic course nurses, and students.
  • Develop good working relationships with nursing colleagues and the multi-disciplinary team.
  • Promote quality care and non-discriminatory practice, acknowledging patients' personal beliefs and identity.

Person Specification

We are looking for individuals with a strong clinical background, experience in palliative care, and a passion for telemedicine. Essential qualifications include a relevant nursing or AHP qualification, non-medical prescribing or advanced clinical assessment, and current registration with the NMC or HCPC. Desirable qualifications include ALS, AIM, or ALERT courses, a first degree in a relevant subject, and palliative care modules.
