Job Title: Qualified Social Worker - Adults Support Role
We are seeking a qualified social worker experienced in working with adults learning disabilities teams. If you possess the necessary skills and qualifications, we encourage you to apply.
Key Responsibilities:- Efficiently manage your caseload.
- Conduct thorough reviews and assessments for vulnerable adults.
- Cultivate strong relationships with other agencies for effective collaboration.
- Promote service users' self-determination, choice, and independence.
- Provide support to service users to maintain safe and independent living situations.
- Evaluate and review existing service users residing in care homes.
- Assess residential, nursing home, or day centre placements.
- Develop collaborative relationships with other professionals to achieve positive outcomes.
Requirements:
- HCPC Registration is a necessity.
- Post-qualifying experience of 1-2 years within the UK Social Care sector.
- Enhanced DBS/CRB clearance is required.
- Eligibility to work in the UK is fundamental.
