Key Responsibilities:
• Provide emotional, medical, social, and spiritual support to patients and their families.
• Conduct thorough assessments and develop personalized care plans tailored to each patient's needs.
• Work collaboratively with healthcare professionals to deliver high-quality care.

Requirements:
• NVQ/QCF Level 3 in Healthcare or equivalent qualification.
• Relevant experience in palliative or end-of-life care, preferably in a home-based setting.
• Valid UK driving license and access to a vehicle for travel between patients' homes.
• Proficiency in using IT systems for remote working, including laptops and other digital tools.

• Competitive salary, estimated at £26,500 - £31,000 per annum, depending on qualifications and experience.
