We operate from two main sites – King George Hospital in Goodmayes and Queen's Hospital in Romford – offering a range of services, including neurosciences, cancer care, and emergency departments. Our outpatient facilities are also available at Brentwood Community Hospital, Barking Hospital, Loxford Polyclinic, and Harold Wood Polyclinic.

The Role You'll Be Applying For

This exciting opportunity has arisen for a Band 7 Macmillan Personalised Stratified Follow-Up (PSFU) CNS to cover 12 months maternity leave, starting in October 2024. As a key member of the Personalised Cancer Care Team, you will be responsible for leading on the implementation of PSFU for Breast, Colorectal, and Prostate cancer patients. This full-time position involves supporting patients to enable them to self-manage and gain confidence following their cancer treatment.

Main Responsibilities
  • You will act as the expert and superuser for the Remote Monitoring System (RMS) module of the Somerset Cancer Registry, developing protocols for remote monitoring and supported self-management in line with regional and national best practices.
  • You will be responsible for enrolling patients, monitoring patients on a stratified follow-up pathway, and escalating concerns where necessary.
  • You will cultivate strong links with appropriate multidisciplinary teams to identify suitable patients and promote the uptake of RMS, educating others in using the system to meet expected KPIs and targets.
Your Qualifications and Experience
  • We are looking for an experienced CNS with formal presentation skills and the ability to present complex information effectively.
  • A proven track record of influencing stakeholders and contributing to project management would be highly beneficial.
  • Familiarity with Somerset Cancer Registry and experience working with the Personalised Care Agenda are desirable qualities.
