About the Role

We are seeking a skilled Community Support Specialist to join our team, working closely with clients who require holistic support. This role involves conducting thorough assessments to develop comprehensive support plans, leveraging partnerships with other departments and external providers to equip and empower clients towards securing permanent accommodation.

Salary

£30,000 - £40,000 per annum (dependent on experience)

Job Description

This position entails:

  • Undertaking client assessments to identify support needs and developing tailored support plans
  • Liaising with colleagues from various departments and external agencies to ensure seamless service delivery
  • Fostering strong relationships with clients to promote trust and confidence
  • Collaborating with the team to monitor progress and adjust support strategies as needed

Required Skills and Qualifications

To be successful in this role, you will possess:

  • Excellent communication and interpersonal skills
  • A deep understanding of the complexities surrounding housing, mental health, addiction, and employment
  • Experience in assessment and support planning
  • Ability to work collaboratively within a team environment
