Job Description:

We are seeking a skilled and experienced Care Co-ordinator to join our multidisciplinary team in Bradford District Care NHS Foundation Trust.

The ideal candidate will possess a professional qualification in mental health nursing (RMN/RLDN) and degree-level education, with experience in care coordination and working with adults and/or young people and children with serious mental health problems.

This is a band 6 role that requires extensive travel across the district. The successful candidate will be responsible for assessing, planning, and providing packages of care for individuals and their families/carers experiencing first episode psychosis.

In addition to the core responsibilities, the job involves supervising junior team members and colleagues, as well as participating in training and implementing sustainable quality improvements across all service areas.

The salary for this position is estimated to be around £45,000 per annum, depending on experience.
