We are seeking a dedicated and compassionate Registered Adult Nurse to join our community health team at Epsom and St Helier University Hospitals NHS Trust. As a Band 5 Community Nurse, you will play a vital role in providing high-quality care to patients in their homes, care homes, and through in-reach services in hospitals.

About the Role

As a Community Health Nurse, you will be responsible for:

  • Providing ongoing care and support to patients in their communities
  • Deploying the most appropriate staff members in a coordinated and timely manner to best utilize community resources
  • Reducing the risk of admission for patients
  • Working closely with community hospitals and specialist teams

You will be part of a dynamic team that serves the Surrey Downs area, working with GP federations, community hospitals, and specialist teams to provide comprehensive care to patients.

Requirements

To be successful in this role, you will need:

  • Registration with the NMC as a Registered Adult Nurse
  • A valid driving license to drive within the UK
  • Community experience and the ability to assess, deliver, and evaluate quality care
