Hospital Discharge and Reablement Specialist

We are seeking an experienced social worker to join our team as a Hospital Discharge and Reablement Specialist. This is an exciting opportunity to make a real difference in the lives of adults who require hospital discharge and reablement services.
Main Responsibilities:
- Conduct assessments to identify the needs of patients and develop personalized care plans.
- Work closely with healthcare professionals to ensure that patients receive the necessary support and resources.
- Provide advocacy and support to individuals and their families, promoting independence and autonomy.
- Develop and maintain relationships with local service providers to ensure access to necessary resources and services.
- Contribute to the development of policies and procedures related to hospital discharge and reablement.
The ideal candidate will have experience in hospital discharge and reablement, excellent communication skills, and the ability to work effectively in a fast-paced environment. The salary for this position is approximately £48,750 per annum, depending on experience.
