**About the Role**

We are looking for an exceptional Specialist Community Health Practitioner to join our team at East Suffolk and North Essex NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients in the community.

The ideal candidate will have a strong background in health care, with experience in working with patients who require specialist care. They will be confident in their ability to assess patient needs, develop and implement care plans, and provide education and support to patients and their families.

We offer a competitive salary of £41,659 - £53,489 per annum, depending on experience. In addition, we offer a range of benefits, including:
