Key Responsibilities
- Provide personalized support to adults with learning disabilities, focusing on their individual needs and goals.
- Work collaboratively with colleagues and external partners to ensure seamless support delivery.
- Develop and maintain strong relationships with individuals, their families, and healthcare professionals.
- Contribute to the development and implementation of personalized support plans.
- Participate in ongoing training and professional development to enhance skills and knowledge.
