Strengths-Based Social Care Professional

We are looking for a highly motivated and experienced individual to join our team as a Strengths-Based Social Care Professional.
The successful candidate will have a strong understanding of strengths-based approaches and the ability to apply them in a variety of settings.
About the Role:
- As a Strengths-Based Social Care Professional, you will work with adults who have complex or fluctuating needs, using a strength-based approach to provide intensive support and reduce risk.
- You will collaborate with other professionals to ensure a multi-agency approach that enables adults and their carers to live independently.
- Your role will involve representing service users' views at various meetings, exchanging information, making decisions, and reviewing progress.
- You will establish and maintain excellent relationships with other agencies and the local community, signposting when necessary.
- Identify potential deprivation of liberty and ensure the least restrictive means of care provision before initiating legal proceedings.
- Maintain accurate records in accordance with the Adult Services' Recording Policy.
- Prepare and participate in reflective practice and supervision.
- Maintain personal and professional development to meet the changing demands of the job, participating in suitable training activities, and encouraging and supporting staff in their development and training.
- Undertake other duties, training, and/or hours of work that may be reasonably required and consistent with the general level of responsibility of this job.
- Conduct and review assessments and support plans for adults and their carers to build a good life within their community.
Key Responsibilities:
- Deliver strengths-based interventions to promote independence and well-being.
- Develop and implement effective assessment and support plans.
- Provide guidance and support to colleagues and other stakeholders.
Requirements:
- A social work qualification or equivalent.
- Registration with the relevant health professional council/governing body (Social Care England).
