Key Responsibilities

We are looking for a highly skilled and experienced Community Wellbeing Coordinator to join our team at Bradford District Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to our patients.

Your key responsibilities will include promoting better understanding and knowledge of mental health and wellbeing within primary care, taking a lead on providing specialist assessments, and providing effective signposting to relevant agencies and services.

You will work closely with secondary mental health services to support effective transitions of service users from secondary mental health services to primary care.

We offer a competitive salary of £41,000 - £53,000 per annum, depending on experience, plus a range of benefits including relocation expenses up to £8,000, increased mileage rates for > 3,500 miles, and a wide range of health and wellbeing support packages.
