About the Job:

Carecymru is seeking highly skilled individuals to join our Assessment Team in Llanelli and Ammanford. As a key member of our team, you will be responsible for assessing the needs of individuals recently discharged from hospital or requiring urgent care, creating person-centred care plans and risk assessing properties. You will work closely with external agencies to ensure service users receive necessary support.

What We Offer:
A competitive salary of £25,000 - £30,000 per annum, training opportunities, DBS payment, MOT payment, social care registration payment, and career progression opportunities are available to successful candidates.
