Job Description

We are looking for a highly motivated Transition Advisor to join our team at Northamptonshire Children's Trust.

This is an excellent opportunity to make a positive impact on the lives of young people in Northamptonshire, supporting them in their transition to adulthood and helping them achieve their full potential.

The ideal candidate will have substantial experience of working with children and young people up to the age of 21 and 25 if they are in higher education in either a voluntary or professional setting.

We offer a competitive salary of £32,500 - £37,000 per annum, depending on experience, plus a range of benefits including 28 days annual leave and a local government pension scheme.

About the Role

The Transition Advisor will work closely with young people aged 18-25 years who have ceased to be looked after by the local authority and are eligible for a service as per the Children Leaving Care Act 2000 and/or The Children and Social Work Act 2017.

The successful candidate will participate in the assessment and preparation of the Pathway Plans for those who are eligible, working closely with the Practice Manager to ensure effective supervision and case discussion.

Key Responsibilities:

  • To provide support to young people in achieving their aspirations regarding education, accommodation, health, and social relationships.
  • To work closely with the Practice Manager to ensure effective supervision and case discussion.
  • To participate in duty rota to meet the needs of allocated young people.

Requirements:

  • Substantial experience of working with children and young people up to the age of 21 and 25 if they are in higher education in either a voluntary or professional setting.
  • Ability to work flexible hours as required to meet young people’s needs, including some evening work and visits out of the county.
  • Excellent communication and interpersonal skills.
