Job Description
Main Responsibilities:
- Providing clinical care to patients with various needs, including wound management, palliative care, and symptom control.
- Working collaboratively with the multidisciplinary team to support patients in remaining at home and avoiding hospital admission.
- Promoting self-care and independence among patients.

Salary and Benefits
The salary for this role is approximately £30,615 - £41,787 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including free parking, mileage rates, and a robust preceptorship program for newly qualified staff.
Person Specification
To be successful in this role, you will require:
- A Registered Adult Nurse qualification or equivalent.
- Experience in community nursing or a related field.
- Excellent communication and interpersonal skills.
