You will support healthcare professionals in delivering developmentally appropriate care for young people, mapping current pathways and implementing gap analysis using national guidance.
Main Responsibilities
* Foster working relationships with Children's and Adult services professionals to develop effective transitional pathways and provide support during complex transitions.
* Collaborate with young people and families to ensure a cohesive and coordinated transition process, maintaining effective liaison with central transition teams in adult services to overcome barriers jointly.
* Develop and provide suitable transition and adolescent health training packages for various staff groups within the Trust, ensuring all specialities have joint transitional pathways with adult services.
* Ensure the trust's transition-related policies and pathways are up-to-date and implement national guidance as required.
