Clinical Lead Specialist

This exciting opportunity is your chance to make a real difference in the lives of young people and their families. As our ideal candidate, you will develop partnerships with professionals from adult and paediatric specialities, ensuring that transitional pathways are cost-effective, safe, and sustainable. You will support healthcare professionals in delivering developmentally appropriate care for young people, mapping current pathways and implementing gap analysis using national guidance.
Main Responsibilities
* Foster working relationships with Children's and Adult services professionals to develop effective transitional pathways and provide support during complex transitions.
* Collaborate with young people and families to ensure a cohesive and coordinated transition process, maintaining effective liaison with central transition teams in adult services to overcome barriers jointly.
* Develop and provide suitable transition and adolescent health training packages for various staff groups within the Trust, ensuring all specialities have joint transitional pathways with adult services.
* Ensure the trust's transition-related policies and pathways are up-to-date and implement national guidance as required.

* Teaching/mentorship qualification
* Current registration with NMC
* Degree or Diploma Level Education as Registered Nurse (RN Child or RSCN)
The estimated annual salary for this position is £55,000 - £65,000.
