About the Role

Windmill Surgery is seeking a skilled Practice Nurse to support our team in providing high-quality care to our patients.

The successful candidate will work closely with the general practice team to deliver policy and procedures, and provide nursing care as required.

Main Responsibilities

  1. To manage patients with asthma and COPD conditions, providing education and support to enhance their health and wellbeing.
  2. To work collaboratively with the IPC lead to implement audits, training, and evidence of compliance with the practice manager.
  3. To develop and embed an effective care management programme for frail and housebound patients registered at the practice.
  4. To assess patient needs and provide appropriate healthcare, incorporating evidence-based practice.
  5. To develop and implement health promotion and wellbeing programmes.

Person Specification

Essential Qualifications

  • Registered Nurse
  • Postgraduate diploma or degree in Asthma and COPD

Desirable Qualifications

  • Postgraduate diploma or degree in other chronic disease management
  • Family planning qualification

Essential Experience

  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working autonomously
