**Job Description:** Wellbeing Support Worker
**Estimated Salary:** £28,000 - £40,000 per annum
This exciting opportunity has arisen to work within the Adult mental health community services at a time of innovation and service development. As a Wellbeing Support Worker, you will be involved in helping to adapt the existing service within the new transformation process.
You will be responsible for participating and assisting in the delivery of care/services to patients, service users, and carers as part of the Core Community Mental Health Team. The post holder will also be part of a daily Core Mental Health MDT referral meeting where all new referrals will be screened and allocated to the relevant Mental health practitioner/named worker.
**About the Role:**
As a Wellbeing Support Worker, you will be assigned to a PCN and will work closely with the PCN Mental Health Practitioner on advice/support for referrals.
Key Requirements:
- Experience in delivering care services to patients, service users, and carers
- Ability to participate in daily Core Mental Health MDT referral meetings
- Practical skills in carrying out tasks as part of service users' care plans
