About The Role

Are you passionate about providing high-quality care and support to individuals in need? As a Support Worker with the British Red Cross, you will play a vital role in enabling adults to live healthier, more fulfilling lives.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with service users and colleagues alike. You will be responsible for providing practical and emotional support, as well as guidance, to service users, with a focus on empowering them to maintain their independence.

Key Responsibilities:
  • Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Assessing service users' needs, completing and following a support plan for the individual.
  • Supporting service users in the weeks after hospital discharge or to prevent readmission.
  • Taking someone to a hospital appointment that they otherwise couldn't get to or be picking up prescriptions and shopping.
Requirements:
  • Good knowledge of services provided by the NHS and Social Care.
  • Ability to make things great, improving service quality for the benefit of users.
  • Professionalism, dealing with queries in a diplomatic and confidential manner.
  • Flexibility, working hours out with the norm.
