Job Description:
As an Advanced Practitioner with Greater Manchester Mental Health, you will play a crucial role in improving service delivery within our Home Based Treatment Team. This exciting opportunity involves attending the University of Salford to participate in the Trainee/Advanced Practitioner course, commencing in September 2024.
You will be part of a dynamic team that supports the clinical and operational teams to ensure high-quality, recovery-focused care is delivered to our service users. On completion of the training, you will be recognized as an expert practitioner in your field, advising the Multi-Disciplinary Team on safe and effective models of care and treatment.
The post holder will work closely with the MDT to develop nurse-led clinics, provide assessment, formulation, and brief interventions of care, including discharge planning within agreed protocols and procedures. You will also take up components of the role as competences are developed and demonstrated in practice.
Main Responsibilities:
- Provide advanced and expert levels of practice within the HBT, leading to improved patient outcomes.
- S Support the clinical and operational team to deliver on service improvements in quality of care and safety through service development, evaluation, and audit.
- Develop nurse-led clinics and provide brief interventions, holding a caseload and leading assessment, formulation, and prescribing of care planning, including discharge planning within agreed protocols and procedures.
- Lead on practice and service developments, demonstrating clinical and practice expertise via interventions, supervision, and modelling good practice.
Required Skills and Qualifications:
- Experience of working with clients with complex needs.
- Compiling protocols and clinical guidance.
- Audit experience.
- Senior clinical nurse-level experience.
