Key Responsibilities:
  1. Work with adults to identify and address their needs, promoting independence and well-being.
  2. Develop and implement personalized care plans, taking into account the individual's unique circumstances.
  3. Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive support.
