Responsibilities:
- Assists individuals with daily living tasks such as personal hygiene, housekeeping, and meal preparation.
- Provides psychoeducation classes on recovery and relapse prevention.
- Coordinates community-based activities, including medical appointments and leisure/recreation/socialization events.
- Participates in developing and implementing client-centered care plans.
- Serves as an advocate for clients within the community and ensures they have access to necessary resources.
- Communicates effectively with clients, families, and healthcare providers to ensure seamless coordination of care.
- Ensures a safe and supportive environment for clients and participates in incident reporting and documentation.
- Adheres to all applicable laws, regulations, and organizational policies.
Qualifications:
- High school diploma or equivalent required.
- 0-1 year of direct care experience working with individuals with mental health, substance abuse, intellectual disability, or physical health conditions.
- Excellent communication and interpersonal skills.
- Strong problem-solving and analytical skills.
- Ability to work independently and collaboratively as part of a team.
- Valid driver's license and proof of automobile liability insurance required.
About Salary:
The salary range for this position is $25-$35 per hour, commensurate with experience. Additional compensation may be provided based on individual circumstances and negotiations during the hiring process.
