Responsibilities:

  1. Assists individuals with daily living tasks such as personal hygiene, housekeeping, and meal preparation.
  2. Provides psychoeducation classes on recovery and relapse prevention.
  3. Coordinates community-based activities, including medical appointments and leisure/recreation/socialization events.
  4. Participates in developing and implementing client-centered care plans.
  5. Serves as an advocate for clients within the community and ensures they have access to necessary resources.
  6. Communicates effectively with clients, families, and healthcare providers to ensure seamless coordination of care.
  7. Ensures a safe and supportive environment for clients and participates in incident reporting and documentation.
  8. Adheres to all applicable laws, regulations, and organizational policies.

Qualifications:

  • High school diploma or equivalent required.
  • 0-1 year of direct care experience working with individuals with mental health, substance abuse, intellectual disability, or physical health conditions.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and analytical skills.
  • Ability to work independently and collaboratively as part of a team.
  • Valid driver's license and proof of automobile liability insurance required.

About Salary:

The salary range for this position is $25-$35 per hour, commensurate with experience. Additional compensation may be provided based on individual circumstances and negotiations during the hiring process.
