Care Assessment Officers play a vital part in our Care and Wellbeing Service, collaborating closely with Sheffield City Council. Your mission is to promote ongoing enablement, helping individuals receiving care reach their full potential for independent living.

Key Responsibilities:
  • Conduct comprehensive assessments and continuous reviews of care needs.
  • Support providers in fulfilling their duties under CQC regulations.
  • Identify and implement the least intrusive services to boost independence.
  • Proactively address life challenges to maintain quality of life.
  • Collaborate with Sheffield City Council, local Primary and Community Health Care networks, and Community Services.
