Are you a skilled healthcare professional seeking a new challenge that combines your clinical expertise with creative problem-solving? We have an exciting opportunity for a Therapy Expert to join our Community Admissions Avoidance Therapy Team (CAATT) at the Royal Gwent Hospital.

The CAATT service provides rapid response to individuals presenting with conditions that impact their daily lives. As a member of our team, you will conduct assessments and interventions in hospital emergency departments and medical admissions units, aiming to facilitate home care and avoid unnecessary hospital admissions.

We are looking for a compassionate and motivated practitioner who can work collaboratively with our multidisciplinary team to provide timely discharges, reduced lengths of stay, and reduced readmission rates through early initiation of therapy interventions.

As a valued member of our team, you will enjoy a Monday-to-Friday service schedule at the Royal Gwent Hospital, with opportunities to work on other sites as needed. Proficiency in Welsh is desirable but not required.

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans
  • Deliver effective therapy interventions to promote independence and well-being
  • Collaborate with healthcare professionals to ensure seamless care transitions

Requirements:

  • Bachelor's degree in Occupational Therapy or related field
  • Experience working in acute settings or community care
  • Strong communication and interpersonal skills
