Patient Care Manager - Adults & Paediatrics Role Overview

HCA Healthcare UK is a leading provider of privately funded healthcare in the UK, with a strong reputation for delivering high-quality patient care. We are seeking an experienced Patient Care Manager - Adults & Paediatrics to join our Case Management Team, working closely with various healthcare professionals to assess, plan, implement, and evaluate care options and services that meet patients' health needs.

About the Role:

The successful candidate will have significant post-graduate experience in Inpatient Bedside Nursing for Adults & Paediatrics, with a proven track record in utilisation review and discharge planning. They will be an excellent communicator, with strong organisational and computer skills, and a dynamic approach to problem solving.

Responsibilities:

  • Lead the development and implementation of patient care plans, in collaboration with multidisciplinary teams
  • Coordinate proactive comprehensive discharge planning activities, ensuring seamless transitions between care settings
  • Act as a liaison between patients, hospitals, physicians, and third-party payers, optimising community treatment/services as required

Requirements:

  • NMC Registered Nurse with substantial post-graduate experience (Adult registered with Paediatric Experience)
  • Experience of utilisation review and extensive discharge planning experience essential
  • Experience as a Senior Staff Nurse/ band 6 or above essential

Estimated Salary: £55,000 - £70,000 per annum + benefits
