Job Summary
We are seeking a highly skilled and dedicated Practice Nurse to join our multidisciplinary team at Three Valleys Hospital. As a Practice Nurse, you will play a vital role in leading and coordinating the physical health needs of our service users, while also supporting your colleagues and promoting good teamwork.
Key Responsibilities
- Monitor observations and assessments to identify any actions that need to be addressed and documented.
- Develop and support physical health interventions used in the clinical area, utilizing knowledge of current best evidence-based practice.
- Engage actively in practice development, evaluation, and audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
- Work collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
- Monitor and maintain Physical Health Dashboards.
Requirements
- A relevant nursing qualification and registration.
- Phlebotomy skills.
- Strong team-working skills.
- A high level of self-motivation and a flexible approach.
- A positive attitude and be naturally caring and compassionate.
- A commitment to high-quality care and to values and evidence-based practice.
- Understand the physical health care needs for individuals who are living with an acquired brain injury.
