Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at The National Autistic Society.

You will be responsible for providing support and guidance to our service users, helping them to develop the skills and confidence they need to thrive in their communities.

Your primary focus will be on delivering high-quality care that is tailored to the individual needs of each person you support.

The ideal candidate will have a strong background in social care, excellent communication skills, and a deep understanding of autism.

Key Responsibilities:

• Developing and implementing personalized care plans
• Providing emotional support and guidance to individuals with autism
• Collaborating with colleagues to ensure seamless delivery of care
• Maintaining accurate records and reporting any concerns or incidents
