Senior Community Support Specialist

The role of the Senior Community Support Specialist is to provide comprehensive support to adults in need. This includes conducting thorough assessments of their needs, developing and implementing individualized care plans, and safeguarding vulnerable adults. You will work collaboratively with multidisciplinary teams, families, and community services to provide a range of support services. In addition, you will advocate for adults' rights, ensuring access to necessary resources, services, and benefits. Regularly review and adjust care plans to respond to evolving needs and situations.
