About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our District Nursing Service in the Anglesey locality. As a key member of our team, you will provide high-quality care and support to patients in their own homes, working closely with registered nurses and other healthcare professionals.

Main Responsibilities

  • Assist registered nurses and practitioners in delivering care to patients, working in partnership with other healthcare professionals, patients, their families, and carers.
  • Deliver patient care as documented in the nursing care plan, carrying out a wide range of duties under the direction of a registered nurse.

Requirements

  • Good general education.
  • Qualification Credit Framework Level 2 (in Care / Health & Social Care) or working towards.
  • Knowledge of consent and patient confidentiality.
  • Understanding of the role and function of Primary Health Care teams.

Desirable Qualifications and Experience

  • Evidence of completion of QCF at Level 2.
  • Understanding of risk management and health and safety issues.
  • Previous experience of working in a care setting.
