Bureau Coordinator

We are seeking passionate professionals to join our team at Carers Resource, specializing in helping individuals transition from hospital to home, emphasizing independence and well-being.

Key Responsibilities:

1. Support individuals discharged from hospital, aiding in their transition to home life.
2. Assess client needs and develop tailored support plans.
3. Collaborate with hospital staff and community health & social care teams.
4. Establish referral pathways to increase community awareness.

Requirements:

1. Previous experience in assessing client needs and developing support plans.
2. Ability to provide support in home settings.
3. Strong interpersonal skills and ability to work independently.
4. Previous experience of inputting information to a database.
