Job Summary:

The role is based in colocated offices with our colleagues from healthcare teams. One team working together to provide seamless care services.

The post involves sourcing care support packages to enable people to go home from hospital where they have health and social care needs.

You will be required to work closely with our Colleagues from healthcare teams to enable discharge from any hospital where the local authority has a duty to meet needs.

Key Responsibilities:

  • To discharge the authority's responsibilities under ASCOF and other relevant legislation with respect to service users with disabilities, vulnerabilities, and eligible care needs.
  • To ensure the council's eligibility criteria, policies, and procedures are followed in relation to service users.
  • To help create packages of support to prevent unnecessary reception of people into care and to assist people to remain in their own home where possible and/or to move to suitable and appropriate residential care.
  • To match service user needs to potential placements or providers.

Essential Requirements:

  • Experience of using computer packages and undertaking administrative tasks is essential.
  • Experience of care standards and relevant legislation and the CQC registration and approval process is essential.
  • Experience of negotiating agreements or contracts is essential.
  • Previous experience of a customer-focused environment is essential.
  • Evidence of continued professional and personal development is essential.

Essential Qualifications:

  • Educated to a high level, A levels as a minimum, is essential.

Additional Information:

  • Working hours: 22 hours per week.
  • The role requires a current standard CRB/DBS clearance.
  • This role will involve weekend working and may cover various shifts.
