As a skilled Nurse Assessor with Sanctuary Personnel Ltd, you will play a vital role in evaluating the healthcare needs of adults with complex conditions.

Location: Birmingham

Salary: £300 per assessment (day rate)

Hours: 37.5 hours per week, Monday to Friday

Contract: Initial 6-month term

Main Responsibilities:

  1. Conduct comprehensive assessments of adult patients aged 18 and above with complex health needs, ensuring timely completion within dedicated timeframes.
  2. Serve as a Case Manager for clients within the Birmingham Solihull ICB area, facilitating effective care planning and service provision to meet individual needs.
  3. Collaborate with healthcare professionals from various sectors to ensure seamless care delivery and present assessment outcomes to BSOL continuing care panels for ratification.
  4. Utilize clinical expertise and experience to guarantee assessments are safe and yield optimal outcomes, adhering to the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.
  5. Maintain partnerships with local community services, primary care trusts, acute hospitals, local authorities, and independent providers.

Requirements:

  • Comply with Health and Safety at Work Act responsibilities.
  • Awareness of safeguarding vulnerable adults' roles and responsibilities.
  • Registered first-level nurse with NMC (RMN or RNLD), considering RGN with relevant mental health and learning disabilities experience.
  • Experience working in learning disability and mental health clinical areas.
  • Undertaking assessments of complex needs.
  • Case management experience.
  • Strong clinical leadership skills yielding high standards.
  • Excellent interpersonal and communication skills.
