Community Care Specialist

We are seeking a highly skilled and experienced Senior Staff Nurse to join our Unplanned Care Team as an A&E Senior Staff Nurse. The successful candidate will have excellent clinical skills, leadership qualities, and the ability to work autonomously in a dynamic community nursing team.
The ideal candidate will have at least two years' experience in community nursing and be able to demonstrate effective management and development of staff. They will also possess strong communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams.

Key Responsibilities- To provide high-quality patient care and support to patients in their homes, working independently and as part of a multidisciplinary team
- To lead and manage the Unplanned Care Team, providing guidance and support to junior staff members
- To develop and implement plans to improve patient outcomes and services
- To work closely with other healthcare professionals to achieve shared goals and objectives
- NMC current registration
- Teaching/Assessor in clinical practice
- V100/V300 prescribing for nursing or equivalent
- Ability to delegate work and demonstrate effective management and development of staff
- Previous experience of community healthcare (District Nursing)
- Understanding and/or experience of managing patients with long-term conditions
- Knowledge of diabetes management, wound care, and leg ulcer management
We offer a competitive salary of £37,338 to £44,962 per annum, pro rata, plus a range of benefits including health and wellbeing support packages, internal and external development and learning opportunities, and flexible working arrangements.
