We are seeking a qualified and experienced Mental Health Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As an Apprentice Health and Wellbeing Practitioner, you will play a vital role in delivering high-quality mental health services to children and young people in schools.
Key Responsibilities- Deliver low-intensity CBT interventions to children and young people in schools
- Work closely with families, school staff, and other professionals to provide a comprehensive service
- Manage a caseload of patients and complete assessments, care plans, and other relevant documentation
- Participate in multidisciplinary team meetings and screening meetings
- Provide consultation and signposting advice to other agencies
- Successful completion of the HEE commissioned 1-year Education Mental Health Practitioner Course
- Registered Nurse, Occupational Therapist, or Social Worker
- Experience of working with children and young people, their families, and others
- Post-registration experience in emotional and wellbeing assessment and care
- Ability to communicate clearly and effectively in English in both verbal and written formats
- 27 days leave plus bank holidays, increasing up to 33 days with long service
