Reablement Support Worker

We are seeking a skilled and compassionate Reablement Support Worker to join our expanding Discharge to Assess (D2A) Team at Candlelight Care.

Job Summary:

As a Reablement Support Worker, you will play a vital role in providing short-term care and reablement services to patients in their own homes, bridging the gap between hospital and home.

Key Responsibilities:

  • Deliver high-quality care and support to patients, promoting independence and well-being.
  • Work collaboratively with the D2A Team to ensure seamless transitions and efficient patient flow.
  • Conduct thorough assessments and develop personalized care plans to meet individual needs.
  • Provide education and support to patients and their families, empowering them to manage their care effectively.
  • Maintain accurate records and communicate effectively with the multidisciplinary team.

Requirements:

  • Previous care experience is desirable but not essential.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Compassion, empathy, and a patient-centered approach.
