About the Role

We are seeking an experienced and motivated District Nurse to join our team at Bridgewater Community Healthcare NHS Foundation Trust. As a key member of our District Nursing Service, you will be responsible for providing high-quality, patient-centered care to individuals and families in our community.

Key Responsibilities
  • Assess and plan care for patients with complex needs, working collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Develop and implement effective care plans, taking into account individual patient needs and preferences.
  • Provide education and support to patients and their families, promoting health and wellbeing in the community.
  • Liaise with other healthcare professionals and agencies to ensure effective service delivery and coordination of care.
  • Conduct comprehensive assessments of patient nursing needs, identifying areas for improvement and developing strategies to address these.
  • Work collaboratively with the District Nurse team leader to coordinate complex and continuing care packages.
  • Maintain accurate records in accordance with NMC standards.
Requirements
  • NMC registered adult nurse.
  • Ability to travel independently across the Trust.
  • Previous community experience and post-registration nursing experience desirable.
