The post holder will be responsible for the clinical assessment of patient needs, development of care programs, and implementation and evaluation of these programs for patients and their relatives.
Main Duties- Communicate effectively with patients and their relatives using a range of methods to ensure a caring, sensitive, and empathetic manner.
- Demonstrate excellent communication skills when dealing with difficult situations, providing clear and concise patient handover to other team members.
- Escalate concerns in a patient's condition to senior nursing team members, medical staff, and initiate referrals to other healthcare team members.
- Report Virtual Ward and departmental affairs to the Senior Nurse Coordinator/Practitioner or the Advanced Clinical Nurse Practitioner and participate in discussions and ward meetings.
- Present a positive, professional image of the organization and the nursing team.
- Participate in Multidisciplinary Team (MDT) meetings and Board Rounds.
- May supervise and assess clinical support workers.
- Prioritize workload to manage the care of a group of patients.
- When rostered in charge, take action to ensure safe staffing levels on each shift.
- Undertake digital assessments of patients to support remote monitoring and patient care.
- Ensure the delivery of holistic patient care, recognizing individual patient needs and those of their relatives.
- Participate in clinical supervision and reflective practice, and participate in teaching and supervision of students, nursing assistants, and other healthcare professionals.
- Maintain accurate records relating to patient conditions.
