Care and Support Team Lead

We are seeking a dedicated and experienced Care and Support Coordinator to join our team at Myn. This role involves leading, supporting, and coordinating a team of Care and Support Workers to ensure high-quality care is provided to service users.

Key Responsibilities:
  • Develop and complete care and support plans and risk assessments tailored to individual service users' needs and abilities.
  • Supervise and support Care and Support Workers to ensure they deliver high standards of care in line with agreed plans.
  • Allocate tasks to Care and Support Workers, monitor their performance, and ensure they are equipped with the necessary knowledge and tools.
  • Conduct risk assessments, including Moving and Handling and environmental/lone worker assessments, ensuring safety for all.
  • Identify training needs and address them to ensure Care and Support Workers are continuously skilled and informed.
  • Ensure regular 1-to-1 and group supervision meetings are conducted in line with the organisation's policies.
  • Oversee the implementation of care plans aimed at promoting and maximising independence for service users.
  • Liaise with health and social care professionals to guarantee the effective delivery of care and support plans.
  • Regularly monitor and audit service user records, ensuring accuracy and relevance.
  • Address unsatisfactory performance and respond appropriately to emergency situations.
  • Assist with personal care needs and medication administration according to care plans, if necessary.
  • Uphold standards of dress and conduct for all Care and Support Workers.

We are looking for someone with proven experience in a care supervisory or coordination role, strong knowledge of care planning and risk assessment processes, leadership and excellent communication skills, and experience managing a team to ensure high standards of care delivery.
