Job Title: Social Care Assessor

We are seeking a skilled and compassionate Social Care Assessor to join our team at Wrexham County Borough Council's Children Services.

About the Role

The successful candidate will work alongside Social Workers to provide information, advice, and assistance as the first point of contact for children in Wrexham. This will involve undertaking 'What Matters' conversations with parents, carers, and children, liaising with partner agencies, assessing care and support needs, and working with families to improve their wellbeing.

Key Responsibilities
  • Undertake 'What Matters' conversations with parents, carers, and children to understand their needs and concerns.
  • Liaise with partner agencies to ensure a collaborative approach to supporting children and families.
  • Assess care and support needs and develop plans to improve wellbeing.
  • Work with families to provide information, advice, and assistance to support their needs.
