Job Title: Advanced Healthcare Practitioner

The Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced and skilled Advanced Healthcare Practitioner to join our Urgent Community Response team. As a key member of our team, you will provide advanced clinical expertise and leadership to our patients and colleagues.

Main Responsibilities:
  • Provide clinical leadership and expertise to the Urgent Community Response team, Care Coordination Centre, Virtual Wards, Early Intervention Model, and Community Nursing Teams.
  • Autonomously provide highly specialized, evidence-based care within the community as part of the integrated health and social care community team.
  • Assess, manage, and treat a range of acute and chronic presentations, demonstrating advanced clinical knowledge and skills.
  • Work in accordance with the Nursing and Midwifery Council/Allied Health Professional (AHP) Code of Professional Conduct.
Person Specification:
  • Extensive relevant up-to-date clinical experience.
  • Evidence of leading service development and participation/experience of mentoring colleagues and students.
  • Understanding of the complex and dynamic needs of a multi-cultural socially diverse community.
  • Knowledge of safeguarding principles (adults and children).
