Job Summary

We have an exciting opportunity for a skilled Therapy Support Worker - Community Care to join our supportive multi-disciplinary Reablement Service in Keynsham, Bath.

As a member of our team, you will work alongside people with a range of physical and psychological conditions and diverse needs within the context of a community-based multidisciplinary team.

Key responsibilities include:

  • Supporting clinicians with assessment and intervention plans
  • Providing rehabilitation and support to promote independence in daily living tasks
  • Encouraging rehabilitation approaches to restore independence
  • Carrying out basic nursing care tasks as necessary
  • Feedback progress of service users and liaising with colleagues and other agencies
