UCLH@HOME Deputy Sister/Charge Nurse

We are seeking an experienced and skilled nurse to join our UCLH@HOME team as a Deputy Sister/Charge Nurse. This is an exciting opportunity to work in a dynamic and innovative service that provides integrated care to patients in their own homes.

The successful candidate will have a strong clinical background and excellent leadership skills, with the ability to work collaboratively with colleagues from acute and community services. They will be responsible for providing direct care to patients, as well as leading and coordinating the team to ensure the delivery of high-quality care.

Main Responsibilities

The post holder will be responsible for:

  • Providing clinical leadership and guidance to the team
  • Coordinating the transfer of patients from hospital to community settings
  • Developing and implementing care plans to meet the needs of patients
  • Working closely with colleagues to ensure seamless care transitions
