Job Summary:

We are seeking a highly motivated and experienced Nurse Practitioner to join our team at OneMedical Group. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and contributing to the success of our medical centre.

Key Responsibilities:

  • Provide comprehensive nursing care to patients with a variety of long-term health conditions
  • Conduct health assessments, screenings, and interventions to promote health and prevent illness
  • Develop and implement individualized care plans to meet the unique needs of patients
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

  • Registered with the Nursing and Midwifery Council (NMC)
  • Degree in Nursing or related field
  • Substantial experience in nursing, preferably in a primary care setting
  • Evidence of ongoing professional development and continuing education
  • Minimum of 1 Long Term Condition diploma
  • Knowledge of patients with long-term conditions and experience in managing their care
  • Familiarity with Patient Group Directions (PGDs) and associated policy
  • Ability to produce accurate, contemporaneous, and complete records
