We are seeking a skilled Nurse or Allied Health Professional to join our team as a Dementia Practice Educator (DPE) at Betsi Cadwaladr University Health Board.

This exciting opportunity focuses on preventing deconditioning and contractures, and optimising positioning amongst residents living with dementia in selected care homes. Initially, four care homes will be supported within a specific locality for 18 months, with the option to discuss the base location during the interview process.

The successful applicant will identify and support care home residents to create individualised deconditioning support plans, prevent admissions, and maintain existing prevention practices. They will also plan and deliver learning opportunities for staff across care homes, secondary care, and community hospital settings, promoting effective positioning and prevention of deconditioning/contractures.

A strong organisational and time management skillset is essential for this role, alongside excellent clinical and leadership abilities. Significant experience in dementia care is required, as well as a passion for improving dementia services and pathways.

The ideal candidate will possess a unique blend of skills, including exceptional communication and interpersonal skills, the ability to work flexibly, and a commitment to delivering high-quality patient care.
