Your Responsibilities:

• Conduct comprehensive assessments to determine functional abilities and identify areas requiring intervention.
• Lead and facilitate engaging group therapy sessions, promoting social interaction and skill development.
• Work closely with a network of third-sector organizations, community teams, and support agencies to ensure seamless care coordination.
